Mental Health Services In Swansea

The Older Peoples Mental Health Service provides assessment, support and treatment to individuals experiencing mental health difficulties, including Dementia. The Older People’s Mental Health Community Service comprises of the Mental Health Primary Care Team, Mental Health Secondary Care Team and Care Home In-reach Team. The teams consist of a range of professionals including; Consultant Psychiatrists, Medical staff, Community Mental Health Nurses, Health Care Support Workers, and Occupational therapists, Physiotherapists, Psychologists, Mental Health Social Workers and Dementia Service Coordinators.

Mental Health Primary Care Team

The aim of this team is to work with people whose functional mental health needs are mild to moderate, and can be addressed by short-term outcome-focussed interventions and/or signposting to available services. The Team operates under Part 1 of the Mental Health Measure.

The Memory Assessment Service aims to provide timely diagnosis and ongoing review/monitoring of those on medication for dementia whose condition is stable. Anyone recently diagnosed with a dementia can access a 7 week information programme which offers education and practical advice about living with a dementia diagnosis. In addition to this we offer Cognitive Stimulation Therapy and Emotional Well-being groups. Every month each site also holds a very well attended carer’s meeting.

The Primary Care Assessment Team is a five day a week community-based service operating Monday to Friday from 8.30am to 5pm.

The Team is based over two sites within the Swansea Area, at Ty Garngoch (01792 531200) and at Westfa Day Hospital (01792 643537).

Community Dementia Team

The Primary Care Dementia Support Team in Swansea provides a first point of contact for anyone concerned about changes to their cognitive health. The team consists of two Senior Dementia Practitioners and six Dementia Support Workers who are now working with the five GP led Health Clusters in Swansea.

If you or anyone you know is experiencing changes to their memory, please either speak to your GP about putting you in touch with the Community Dementia Support Service or contact us yourself via the Social Services, Common Access Point; Tel: 01792 636519. We will arrange for one of our friendly Dementia Support Workers to come and talk to you about your concerns and help you decide on what to do next.

Admiral Nurses

Admiral Nurses are specialist dementia nurses who give expert practical, clinical and emotional support to families living with dementia to help them cope. The two Swansea nurses will work closely with social workers, community nurses, mental health teams and GPs, providing specialist community-based care for dementia patients and their families.
