Social Services in Swansea offer support those people with dementia and their carers. They work together with local health services and the voluntary sector.

Common Access Point – CAP Information

In Swansea, the local authority aim to provide adults and their carers with the right support at the right time by the right person. They do this initially through the ‘Common Access Point’, which is a place that anyone can contact, to access information and advice on health and social care.

The Common Access Point team has ‘Access and Information Assistants’, who will talk to you about your situation and provide you with information, advice and signposting. The Access and Information Assistants can also put you in touch with community based services, such as local groups or charities. Often this may be all the help you need.

Emergency Help

You can contact Social Services in an emergency – the CAP number during office hours 01792 636519 or out of hours the emergency duty team on 01792 775501 or email edt.edt@swansea.gov.uk

About the Emergency Duty Team
This team deals with out-of-hour emergency situations that cannot safely be left until the next working day, and will provide information and advice to keep the situation safe until the next day.
Opening hours:
5.00pm – 1.00am Monday to Thursday.
4.30pm – 1.00am Friday.
Weekends and bank holidays: 9.00am – 1.00am.
One person remains on duty from 1.00am until 9.00am for the most serious emergencies only.

Carers ID/Emergency Card

Swansea Carers Centre have developed a new Carers ID/Emergency Card following feedback from carers during the Covid-19 pandemic. The card has a dual purpose because it also serves as an Emergency Card. Carry this card in your purse or wallet as it identifies you as a carer, shows your name and an emergency telephone number to call to alert someone that the person you care for needs help.
