Social Services in Swansea offer support those people with dementia and their carers. They work together with local health services and the voluntary sector.

Common Access Point – CAP Information

In Swansea, the local authority aim to provide adults and their carers with the right support at the right time by the right person. They do this initially through the ‘Common Access Point’, which is a place that anyone can contact, to access information and advice on health and social care.

The Common Access Point team has ‘Access and Information Assistants’, who will talk to you about your situation and provide you with information, advice and signposting. The Access and Information Assistants can also put you in touch with community based services, such as local groups or charities. Often this may be all the help you need.

Emergency Help

You can contact Social Services in an emergency – the CAP number during office hours 01792 636519 or out of hours the emergency duty team on 01792 775501. If you are not sure who to call visit the Choose Well website. This will help decide if you or a person with dementia need medical attention.

Carers Emergency Card Scheme

Register with this scheme to get a card that will identify a carer in an emergency should you be involved in an accident or emergency. This will identify that there is someone who relies on you for their care and assistance. There is space for two telephone numbers on the card so that they can be contacted in an emergency.
