Mobile Dementia Hwb Support Worker

Funded by UK Government and Community Foundation Wales, Dementia Friendly Swansea have 2 Part-time vacancies for Mobile Dementia Hwb Support Workers.

1x 3 days a week (including Fridays), 1x 4 days a week (including Fridays and Saturdays).

Both roles will work independently at various venues across Swansea, usually from 09;00-17:00 on set days

No home working available. Own transport is required

You will be responsible for taking the information that we have in the Dementia Hwb in the Quadrant Shopping Centre out to a wider audience and to follow-up with visitors as appropriate. You will be a warm, friendly and empathetic listener as well as highly knowledgeable about local support and services for people affected by dementia. Working as a key part of a small, innovative team your drive to improve quality of life for others will likely shine through in sharing great ideas and helping to develop this new project.

Mobile Dementia Hwb Support Worker

As Mobile Dementia Hwb Support Worker responsibilities will include but is not limited to:

  • You will be responsible for taking the information available in the Dementia Hwb out to non-city centre locations such as community centres, cafes and other venues
  • You will become the named contact for assigned visitors that you meet through the Mobile Dementia Hwb locations and will continue to navigate and support them through the Dementia journey all the way to End of Life
  • Work as a team with all staff in the Mobile and main Dementia Hwbs, ensuring referrals to other organisations are followed-up and all visitors to the Mobile Hwb have a named contact for ongoing support
  • Ensure that all visitor communication you have is recorded on our contact management system
  • Make regular check-in phone calls to previous visitors to provide ongoing support
  • Production of weekly and monthly statistics of Mobile Hwb visitors and their queries
  • You will be responsible for liaising with venue staff to ensure smooth operational running of the Mobile Dementia Hwb
  • You will be responsible for ensuring that the general look and feel of the Mobile Hwb/Information and literature is of a high standard
  • Carry out any other activities as reasonably asked of you by the Project Lead

Requirements and skills

  • Excellent communication skills both written and oral
  • Demonstrable skills in working with and assisting people with a shown interest in the community and in particular Dementia
  • Confident use of MS office (Word, Excel)
  • Experienced to be able to multi-task and prioritise workload
  • Confident talking to people in the community and ability to respond to changing situations
  • Excellent administration skills with a proven ability to be able to produce accurate and timely

    data reports
  • Demonstrable skills of working as part of a team that has achieved success
  • Driver with own transport suitable for carrying folding table and leaflets
