Age Cymru West Glamorgan are a charity who offer a range of services to support local people, families and carers. They have launched a new free service to help support people with dementia and their carers.
They will create a personalised dementia support plan combining information and contacts, tailored to your needs.
The plan will include details of local services, community groups, home support, local transport, useful websites, benefit/legal advice, support for carers and more, to ensure you are empowered and informed about the support that is available.
The service is free and is available to older people in the Swansea, Neath and Port Talbot Area.
